Part of #23 (tracked in dit-operator). Doc-only: update INSTALL.md dito command table to canonical commands (tenant-converge, ditc-grant-access, tenant-init, recover, etc.) + skill counts (ditc 12, dito 22), bump version headers (ditc v0.5.0, dito v0.8.4), add Troubleshooting + Uninstall sections, delete the obsolete INTERNAL-TEST-INSTALL.md.
